Applying for a Dog

If you are enquiring on behalf of a person who might want a dog or want to know more before you complete an application form, your telephone call is welcome.

If you would like to apply for an Assistance Dog please fill in the application form and return to ASDOG. Once the application form is received, you will receive a telephone call and if you are a suitable candidate a more detailed interview will take place in your home.

As ASDOG continues to offer support and help for the lifetime of the dog and recipient team we can only assist people who live in Sydney and no more than two hours travel to the north, west and south.
